Phases of Rehabilitation
Your Journey with Proactive Physiotherapy & Wellness
Reception and First Welcome
When you arrive for your first appointment, our reception team will be the first to greet you. They help you settle in, guide you through the check in process, and coordinate your future appointments. Throughout your rehabilitation, they remain your consistent point of support for anything administrative or scheduling related, ensuring your experience is smooth from start to finish.
Initial Consultation
Your first session begins with a detailed assessment by one of our physiotherapists. At Proactive, this specialist is responsible for understanding your full clinical picture and mapping out the treatment pathway that best fits your needs and goals.
They will guide you from the very first evaluation to diagnosis, planning, and ongoing progress reviews. Regular check ins are part of this process, helping us refine your programme as your body heals and adapts.
Your Rehabilitation Specialist
Following your initial consultation, you will begin working closely with your rehabilitation specialist. This is the clinician who will walk with you through your sessions, motivate you, adjust exercises as needed, and ensure every step of your rehabilitation aligns with the plan created during your assessment.
They serve as your day to day expert inside the clinic, ensuring you feel supported, informed, and confident throughout your recovery.
Follow Up Assessments
After your first block of sessions, you will meet your physiotherapist again for a progress review. They will evaluate how your body is responding, adjust your treatment plan where needed, and determine whether additional sessions, different techniques, or further assessments are required.
This stage ensures that your recovery remains personalised, precise, and paced correctly for your body.
Functional Assessment Testing
At various points in your rehabilitation, we may carry out functional tests designed to measure strength, mobility, endurance, balance, and quality of movement. These tests allow us to track progress objectively and adapt your programme based on real performance markers.
For athletes, these evaluations are especially important when determining readiness for return to sport.
On Field Rehabilitation
When appropriate, rehabilitation progresses from clinic based sessions to on field work. This applies to athletes, weekend warriors, and everyday individuals alike.
On the field, we recreate real movement patterns and task specific drills that mirror your sport or daily activities. The focus is on rebuilding confidence, refining movement quality, and restoring the dynamic function your body needs to return safely to full activity.
Every exercise is tailored to your unique movement patterns, ensuring a gradual and safe transition back to normality.
Ongoing Care and Long-Term Support
At Proactive, rehabilitation doesn’t have a strict end point. Instead of a formal discharge, your physiotherapist works with you until you feel confident in your progress and ready to transition into maintenance, performance training, or occasional check ins.
Before you wrap up your active rehabilitation sessions, your clinician will review your improvements, answer any concerns, and guide you on what to continue at home or during training.
Whether you choose to return weekly, monthly, or only when needed, our team remains available for support, tune ups, and injury prevention because long term health and performance don’t end with recovery.
